Your exact checklist may change based on your circumstances. Start with the following:
- The child’s full name, date of birth and sex
- Hospital, home address, parish and district where the birth occurred, if known
- Mother’s full name and maiden name
- Father’s full name, if recorded or known
- Registration code, number, date or place, if available
- Applicant’s name, address, relationship to the person named on the record and contact details
- Preferred delivery or pickup location and the reason for applying
The Office of the Registrar-General advises that accurate record information improves the chance of prompt processing. Additional searches or record-correction procedures are separate.

What should I send first?
Start with a short description of the request, the relevant deadline and a list of the documents you already have. Do not send original documents unless collection has been specifically agreed.
Are official fees included?
Government, registry, delivery and third-party professional fees are separate unless the written quotation expressly includes them.
